Investigators have concluded that a fatal plane crash on June 8, 2008, was caused by the pilot's failure to maintain control of the airplane, causing the plane to stall and crash.
Investigators also found that the 86-year-old pilot should not have been flying due to his vision and heart problems. Therefore, investigators have determined that the physician who signed off on the pilot's airman medical certificate a year earlier, also contributed to the crash that killed the pilot and his five passengers.
The National Transportation and Safety Board report states that the pilot provided false documentation to his doctor, but that his vision problems should have been easily detected.
